Summary::
A Licensed Practical Nurse (LPN) working in our Skilled Nursing facility is responsible for the daily nursing care needs of our residents. This position is a valuable clinical team member contributing to the smooth, organized and efficient operations of the nursing department.
Responsibilities:
Provide primary nursing care to residents with varying backgrounds and medical histories
Assists with new admissions
A Licensed Practical Nurse (LPN) will monitor and communicate changes of condition to provider
Provide medical intervention as needed for emergent needs
Communicate needs of resident to outside providers including but not limited to: Primary Care Provider, Home Health Agency, Pharmacy, etc.
As a Licensed Practical Nurse (LPN) you would communicate resident condition to family, and or responsible party members
Implement and follow Primary Care Providers orders
Other Duties as Assigned
REQUIREMENTS:
Licensed Practical Nurse (LPN) Licensure in the State or practice
